Hospitalization Procedures

What you need to submit

Hospitalization Application Form, Pledge/Consent Form, Hospital gown,towels,daily necessities and 
diapers rental Form

※Depending on the type of hospitalization, you may be required to submit additional documents.

A personal seal from each person and their family member is required (as it is required for admission procedures). A mental health and welfare worker (social worker) will explain the details.

What you need to present

Health Insurance Card or Myna Health Insurance Card

Please be sure to present your health insurance card or My Number health insurance card at the general reception desk at the time of your first visit or admission.

  • Atomic Bomb survivor's certificate
  • Welfare medical care recipient certificate
  • Public assistance recipient certificate, etc.

Please present the above if you have it.

  • Our hospital recommends using your My Number health insurance card. We appreciate your cooperation.
  • By having a My Number insurance card, you will no longer need to apply for a Eligibility certificate for ceiling-amount application.
  • Please present your health insurance card (My Number health insurance card) once a month.
  • If there are any changes after that, please notify the general reception promptly.
Request to switch to a standalone/remote area insurance card (for those living on remote islands or outside the prefecture)

You may also receive medical treatment or examinations at other medical institutions other than ours, so please switch your health insurance card to a stand-alone or remote area insurance card and leave it at the general reception desk.
